Can psychedelics help with anxiety or PTSD?

Early-to-late-stage trials report symptom reductions for some psychedelic-assisted approaches — LSD-based DT120 in generalized anxiety, MDMA-assisted therapy in PTSD — but none is FDA-approved for anxiety or PTSD, and the evidence has real limits. The FDA declined to approve MDMA-assisted therapy for PTSD in August 2024, and a resubmission reported in August 2026 has not been publicly confirmed by the agency.

This page summarizes published and company-reported results as of October 20, 2026. It describes research on screened, closely monitored study participants; it cannot tell you what would happen for you, and it is not medical advice. Anxiety and PTSD are conditions for a licensed clinician to evaluate.

Generalized anxiety: LSD-based DT120 (formerly MM120)

The most advanced anxiety program is Definium Therapeutics' DT120 (an LSD formulation, referred to as MM120 in earlier publications) for generalized anxiety disorder (GAD). Outcomes are measured on the HAM-A scale (0–56).

Phase 2b (198 adults, single dose; published in JAMA, 2025): placebo-adjusted HAM-A change at week 4 was −5.0 at 100 µg (95% CI −9.6 to −0.4) and −6.0 at 200 µg (95% CI −9.8 to −2.0); lower doses did not separate from placebo (Wellness Radar summarizing Robison et al., JAMA 2025).
Phase 3 Voyage (214 adults; 100 µg vs placebo): at week 12, HAM-A fell 11.6 points vs 6.2 with placebo — a difference of −5.4 (p<.0001; Cohen's d = 0.81) (HCPLive, Aug. 2026).
Phase 3 Panorama (245 adults; 100 µg, 50 µg, or placebo): at week 12, HAM-A fell 9.8 points at 100 µg vs 4.7 with placebo — a difference of −5.1 (p<.0001; d = 0.64) (Psychiatric Times, Sept. 14, 2026).

Caveats: the Phase 3 figures are company-announced topline results, not yet peer-reviewed publications. Adverse events were common on dosing day — in Panorama, 94.8% of the 100 µg group reported at least one treatment-emergent event versus 62.9% on placebo, most commonly illusions, nausea, and headache — and the company reported no suicidality signal and no drug-related serious adverse events. Definium has said it plans to discuss an NDA with the FDA; no application has been approved.

Anxiety in serious illness: psilocybin

A small randomized crossover trial of 29 treated patients with life-threatening cancer compared a single psilocybin dose (0.3 mg/kg) with niacin as an active control, both with psychotherapy. Roughly 60–80% of participants maintained clinically significant reductions in anxiety or depression at the 6.5-month follow-up. Transient adverse events included blood-pressure and heart-rate elevations (76%), headache (28%), transient anxiety (17%), nausea (14%), and transient psychotic-like symptoms (7%); no serious adverse events were attributed to either substance (Ross et al., J Psychopharmacol 2016). It is a small, single-site study — useful as a signal, not proof.

PTSD: MDMA-assisted therapy

In the Phase 3 MAPP2 trial (104 participants), the average CAPS-5 PTSD-severity score fell 23.7 points with MDMA-assisted therapy versus 14.8 with placebo plus therapy at 18 weeks — a difference of −8.9 (95% CI −13.7 to −4.1; p<.001) (MDedge on Mitchell et al., Nature Medicine 2023).

Even so, the FDA declined to approve it in August 2024, citing the need for more on durability, fuller safety characterization, and steps to reduce bias from participants being able to tell they had received MDMA. Resilient Pharmaceuticals (formerly Lykos Therapeutics) was reported on August 9, 2026 to have resubmitted its application without a new Phase 3 trial; neither the company nor the FDA had publicly confirmed it at the time (Healing Maps, Aug.–Sept. 2026). A review summarized in October 2026 pooled six randomized trials (286 participants, all sponsored by MAPS) and rated the overall certainty of evidence as low; it also noted weak blinding, non-independent replication, and MDMA's physical risks (including hyperthermia, hyponatremia, seizures, cardiovascular effects, and serotonin toxicity) (Psychology Today, Oct. 2026).

What isn't known yet

Whether benefits last beyond the follow-up windows studied, how much expectancy and unblinding inflate results, how outcomes hold up outside specialized trial settings, and how these approaches compare with established first-line treatments. The U.S. Department of Veterans Affairs began an MDMA-assisted therapy trial for veterans with PTSD and alcohol use disorder in May 2026, which is intended to add evidence.

Where access stands today

MDMA and psilocybin are Schedule I federally; LSD is too. Outside clinical trials, none of these is available as an approved treatment for anxiety or PTSD. See psilocybin vs ketamine vs MDMA therapy for current approval status and where psychedelics are legal in the US for state programs.

How a guide fits in

Frequently asked questions

Is any psychedelic FDA-approved for anxiety or PTSD?+
No. None is FDA-approved for anxiety or PTSD. The FDA declined to approve MDMA-assisted therapy for PTSD in August 2024, and a resubmission reported in August 2026 had not been publicly confirmed by the FDA. LSD-based DT120 has reported positive Phase 3 results for generalized anxiety disorder but has no approved application.
What did the Phase 3 anxiety trials of DT120 (LSD) find?+
Company-announced topline results: in Voyage, HAM-A scores fell 5.4 points more than placebo at week 12; in Panorama, 5.1 points more than placebo at the 100 µg dose. These are group averages that have not yet appeared in peer-reviewed publications, and most participants reported dosing-day adverse events.
Why did the FDA decline MDMA-assisted therapy for PTSD?+
In August 2024 the FDA asked for more on durability of response, fuller safety characterization, and steps to reduce bias from participants recognizing MDMA's effects.
Is MDMA-assisted therapy available anywhere?+
Only in clinical trials. It is a Schedule I substance and is not approved for PTSD.
Can Guides Collective treat anxiety or PTSD?+
No. Guides Collective is an informational platform, not a medical provider, and guides are not clinicians. If you are in crisis, call or text 988 in the U.S.
